Conversations in Literacy has a resource titled: The 5 W Questions Plus How Classroom Posters! These are Common Core posters that answer the 5W + How questions! This is great for students in grades 1-3.

Intermediate
Ashleigh has an engaging resource titled: Fraction Avenue! Students get to design a street by using fractions! 

Secondary
Lindsay Perro has a resource titled: Graphing Linear Equations with Color. Students are given 14 equations that have to be written in slope-intercept form. They must then graph them using the indicated color.
